Services

Longwood at Home's low monthly fee provides a broad spectrum of programs and the assurance that quality, cost-effective long-term care services will be available for a fraction of the cost compared to paying separately for such services.

Care Coordination
As a member of Longwood at Home, you will be assigned a personal care coordinator to arrange any services you might require. The personal care coordinator will work closely with you, your family and your physician to have the right services delivered to you in your home, or when necessary, in a facility. In the event of an emergency, a member of the care coordination staff is available to you, 24 hours a day, seven days a week.

Home Inspection
During the first year of membership and every second year thereafter, Longwood at Home will provide a safety and functional inspection of your home. The inspection is completed by a licensed therapist, who may make recommendations to you based on the inspection.

Transportation
Longwood at Home will provide transportation to and from medically necessary outpatient surgery and to some short procedures or hospitalization. Longwood at Home also assists with transportation coordination for routine doctor visits and social needs.

Home Nursing Care
You can receive home healthcare services provided by a registered nurse or licensed practical nurse.

Home Health Aide
If you need assistance with your personal care, such as bathing or dressing, a licensed home health aide will be provided for you in your home.

Companion/Homemaker Services
A companion/homemaker will be provided when you need assistance in your home with such things as supervision of activities of daily living (ADLs) and/or medication reminders. This caregiver also may do cooking, dishwashing, laundry, light housekeeping, and errands, as well as keep you company with conversation and social activity.

Live-In Assistance
If assistance is required 24 hours a day, seven days a week, a live-in option is provided at different coverage levels, depending on the Longwood at Home program you have chosen.

Emergency Response System
For members deemed to be “at-risk”, Longwood at Home will provide an emergency response system with 24-hour coverage.

Adult Day Services
Adult day services are located throughout the community and offer programs provided in a congregate setting for a scheduled number of hours per week. Transportation, meals and activities also are provided by Longwood at Home.

Assisted Living
Assisted living is used when staying at home no longer is possible, but the member doesn't require the medical care that a nursing home provides.

Nursing Care
If your health changes and you no longer can remain in your home or in assisted living, skilled nursing care is included for members. Participating skilled nursing communities provide 24-hour nursing and medical supervision.

Referral Services
You may obtain a referral from Longwood at Home for other services you might require to stay in your home, such as legal advice, financial planning, home maintenance, rental of medical equipment, etc. The member is responsible for paying for these services.

Social and Wellness Programs
Services provided by Longwood at Home include events such as quarterly member meetings, health and wellness seminars and other social events.
