Longwood at Home provides a broad spectrum of services and care in a Member's home with the goal of remaining independent as long as possible. Services similar to those provided by a residential Continuing Care Retirement Community, along with care coordination give Longwood at Home Members a cost-effective and comprehensive solution for remaining at home and aging in place. Six program levels -- Platinum, Gold, Silver, Bronze, Sterling, and Gold Plus, have been designed to offer individualized care at the level a member chooses. All six programs offer the services of a care coordinator, which gives Longwood at Home members and their families a great level of comfort in knowing that needs will be addressed on an ongoing basis.
