Longwood at Home, an affiliate of Presbyterian SeniorCare, understands that for many older adults the ability to remain at home and age in place greatly enhances quality of life. As an active, vital older adult, you might share that same commitment to remaining at home, but perhaps you have concerns about the future.

Longwood at Home's care coordination sets us apart from many other traditional long-term care plans. Members of Longwood at Home will have the benefit of a professional personal care coordinator who builds a relationship and is available to develop a care plan to meet a member’s needs. Whether there is a short-term need or a long-term diagnosis, one call to the care coordinator is all it takes to put a care plan in motion. This assures members that they will have excellent care and the option to maintain their independence. It also provides assurance to adult children that if they're not available to help mom or dad, the care coordinator is able to handle all care-giving arrangements.
Longwood at Home provides six levels of coverage for healthy adults age 60 and over: Platinum, Gold, Silver, Sterling, Bronze, and Gold Plus. Each program offers the opportunity to remain independent at home, a professional care coordinator and access to a lifetime of excellent, affordable care and services.

About Presbyterian SeniorCare
For 80 years, Presbyterian SeniorCare (PSC) has been improving the lives of older adults. This faith-based, not-for-profit organization is Pennsylvania’s first accredited Aging Services Network.
